On some faces, this can lead to an appearance that patients describe as ‘sleepy-looking facial expression” or the appearance of “smaller-looking eyes”. Figure 7, the patient had moderate brow ptosis with a preoperative MRD1 of 3, MFD 0, MCD 4, and VSD 25. Because the patient desired an MFD of 4 to 5 mm, the numbers were inputted into the equation and the recommended new crease, MCD, was to be placed at 8 to 9 mm. After discussing possible outcomes with the patient, an MCD of 9 was decided. The equation predicted a postoperative MFD of 4.5, which was very close to the patient’s actual postoperative MFD of 4 and 3.5 for the right and left eyelids, respectively.

By inhibiting the ability of the blood to flow freely, healing slows. Slower healing increases the likelihood that a patient will experience open wounds or develop an infection. In addition, this puts the patient at a higher risk of skin necrosis (i.e., tissue death). During a functional blepharoplasty, the surgeon lifts a drooping upper eyelid, which restores the patient’s field of view. Through this upper-eyelid incision, the surgeon views the tissues below the surface of the skin to determine which tissues are causing the patient’s upper eyelid to appear wrinkled or puffy. After excising the orbicularis oculi muscle beneath the incision the fat tissue over the top of the tarsus should be removed until the levator aponeurosis can be clearly identified.

Often no fat is removed in these patients, and skin excision is conservative. This skin incision height is often quit low, 3 to 5 mm depending on the preoperative consultation measurements. In older patients with excess upper lid fat, the septum needs to be formally opened to remove preaponeurotic fat. The skin incision should still be kept low, perhaps at 5 to 6 mm at the most. Crease formation should not be high on the levator to avoid a distorted “westernized” look, asymmetry, and ptosis.
